We regret that you cannot download unable application forms as we require to attach a reply paid envelope for your doctor. Please contact Julie or Sylvia and one will be sent to you. Please note that there are different application forms for chaplain, doctor, nurses and helpers. Please use the correct form and if you are in any doubt about the suitability of your medical condition, please contact the office for advice. A medical condition does not preclude you travelling as a carer, but it is important we know the strengths and weaknesses of the team. You may also download the brochure Travel to Lourdes by Jumbulance and an application form to join the 200 Club or Friends of ACROSS or the latest ACROSS accounts. Before you can download any items you will require to have Adobe Reader installed on your PC. This is free and can easily be downloaded from this link. A medical handbook or group leaders handbook can be downloaded from the restricted area for those with password authorisation.
